We at Open Water Medical are among the few eastern North Carolina practices to have achieved the status as a BQIP practice. BCBS recognizes the performance levels of a participating practice in this innovative BCBS program. BQIP builds on the Patient Centered Medical Home program to reward high performing cost effective practices. These practices are recognized for their enhanced services and time spent in providing high quality, cost effective services to BCBS patients.

Open Water Medical is pleased to be a participating practice in the chronic program created by the Center For Medical Services (CMS). Today in the United States the total cost of chronic disease such as cancer, diabetes, pulmonary conditions and mental illness are keeping patients needlessly sick and caught in the web of the health care system. Before this program, most chronically ill patients were left to their own devices in between medical care appointments. CCM allows us to dedicate more time to providing extra services such as medication reconciliation, appointment management and routine checks between appointments to smooth out unexpected and unanticipated problems with patients. 

As a result of the CCM program we are able to effectively offer high risk patients the quality of care that they deserve. As a result of being enrolled in the Chronic Care Management program our nurses will contact the patient monthly to discuss the care plan that has been personalized for the patient. During the call, the nurse will discuss any issues that arise dealing with the chronic condition. Our nurse will ensure that when the patient has to go to the hospital or another facility that the Open Water Medical provider will be made aware so they can provide needed information to avoid any lapse in care that could occur.
